Banns Only Form
This ‘Banns of Marriage Form’ has been developed by the Church of England’s Life Events team to reflect changes to marriage registration introduced in May 2021.
It is designed for the minister or church administrator to use with couples whose banns will be read in the parish, but whose wedding will take place elsewhere.
The Weddings Welcome Form – designed for use with couples who are also to be married in the parish – is also available, combining the required legal information for the reading of Banns with helpful information to complete the Marriage Document and to support pastoral care and service-planning.
The form is available in full-colour and in an ink-friendly black and white version for printing and filling in by hand:

NB: When opening the editable PDF, the form tends to work better when it is opened with Safari and Chrome browsers than when opened with Internet Explorer. If you wish to send this to couples on email, it would be best to send it as a link, rather than as a downloaded attachment, with the instruction to open it using Chrome or Safari for best results. Alternatively, you may prefer to send them the ordinary PDF version, which they would print out and fill in by hand.
